What is Children's Community Health Plan?
Children's Community Health Plan is an HMO for BadgerCare Plus eligible families living in Brown, Calumet, Kenosha, Milwaukee, Outagamie, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Waupaca and Winnebago counties in Wisconsin. Children's Community Health Plan is affiliated with Children's Hospital and Health System.
Does Children's Community Health Plan only cover children?
Children's Community Health Plan covers all family members as long as they are eligible for BadgerCare Plus.
What benefits does the health plan offer my family?
Children's Community Health Plan offers all of the same medical services that BadgerCare Plus covers such as:
Access to a primary care physician
Children's Community Health Plan members choose a primary care physician that helps them stay healthy. Since all members of a family may be covered by CCHP, the plan allows each family member to choose from a large network of providers that include:
- More than 5,000 health care providers.
- More than 25 hospitals.
- Home care and medical equipment providers.
- CCHP Nurseline - nursing advice is just a telephone call away.
