Children's Community Health Plan provides services to BadgerCare Plus eligible adults and children living in the following Wisconsin counties: Brown, Calumet, Kenosha, Milwaukee, Outagamie, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Waupaca and Winnebago.

Missed appointments
It is important that you and your family keep all your health care appointments. If you are not able to keep the appointment, call your doctor's office and let them know as soon as possible. Otherwise, the provider may refuse to reschedule your appointment. |
