Send Us Your NPIs
CCHP would like to receive your NPIs so that we can map them into our systems. We are using the HIPAA COW standard file formats to capture your Type 1 (Individual) and Type 2 (Organizational) NPIs. All you need to do is download the Excel templates and instructions below. When you have finished filling out the files, e-mail them back to us using the address listed.
Type 1 NPIs
Please use the template and links below to send CCHP your Type 1 NPIs. Since we are collecting Type 1 and Type 2 NPI's in different file layouts they do not have to be sent at the same time.

Type 2 NPIs
When you are ready to send us your Type 2 NPIs, use the template and links below. Please send only the Type 2 NPIs that you will be using on your claims to Children's Community Health Plan. We realize that you might be billing claims differently between health plans, so we only want the NPIs necessary for our crosswalk. Based on the NPI mandate, you will receive a separate payment and EOP for each Type 2 NPI you send us. For example, if you have 3 NPIs for a current CCHP vendor number, you will receive 3 separate checks and EOPs.
