Prenatal Care Coordination Program
CCHP is dedicated to promoting healthy birth outcomes. We now provide prenatal care coordination for our members. As the medical provider, you work closely with our members to ensure that quality medical care is provided. In order to provide pregnant CCHP members with appropriate care management services, we require you to complete and submit a notification of pregnancy form. The referral form enables you to make referrals for high risk patients.
Practice Guidelines
The clinical practice guidelines are designed to assist clinicians by providing a framework for the evaluation and treatment of patients. These guidelines have not been developed as, nor should they be construed as, absolute standards or minimal levels of care. The ultimate judgment regarding the application of any specific mode of treatment must be made by the physician in light of the individual circumstances presented by the patient. In other words, these guidelines are not intended to replace the clinical judgment of health care professionals.
The information in these guidelines has been gathered from medical literature and opinion. These guidelines are not a substitute for a provider's judgment.
The following guidelines are reviewed by Children's Community Health Plan's Medical Advisory Committee at least every two years and are updated earlier if there are significant evidence based changes in best practices or updates to reference guidelines.
