17-Alpha-hydroxyprogesterone Caproate (17-P)
Children's Community Health Plan/Dean Health Plan Southeast will provide 17-Alpha-hydroxyprogesterone Caproate (17-P) as a benefit for members who have been identified by their physician as at-risk for preterm delivery.
17-P is now being approved by Wisconsin Medicaid; please follow the link for information on the Pharmacy page of this website for providers to access.

Tapestry Cards - Tapestry Cards present information on culture that can help us become more aware of our patients' and co-workers' views, traditions, and actions. There is information on interpersonal relationships, treatment issues, important holidays and dates, as well as a brief glossary of words and their meanings. While you can use this information as a guide, keep in mind that all people within a culture are not all the same. Be sure to ask your patients and their families about specific beliefs, practices, and customs that may be relevant and important during medical treatment and hospitalization. With respect to co-workers, the same guidance applies to learning more about their beliefs, practices, and customs in order to create respect and teamwork in the work place. These cards are available for our use with permission from the University of Chicago Hospitals.
Access standards

Children's Community Health Plan establishes standards for appointment access and after-hours care in order to ensure timely access to care for members.

The providers in the CCHP network have hours of operation that do not discriminate against BadgerCare Plus Standard or Benchmark members.

Translation services are available if a provider does not speak the member's language.
Primary care provider appointments Regular and routine care appointments – within 30 calendar days. Urgent care appointments – within two calendar days. After-hours care – 24 hours, seven days a week. Office wait times – within 30 minutes.
Mileage access standard for primary care Milwaukee County access standard – two primary care physicians within three miles. All other counties access standard – 1 primary care physician within 10 miles.
Prenatal care appointments High Risk – within two weeks of request for an appointment or within three weeks if the request is for a specific practitioner.
Emergency care – immediate Members are instructed to directly visit the nearest emergency room or call 911 if they are experiencing a life-threatening situation. Members do not need prior authorization to seek emergency services.
Dental care Routine – within 90 days. Emergency – immediately. A dental emergency is defined as uncontrollable pain, swelling or bleeding.
