Newborn Welcome Packets

Children's Community Health Plan wants you to have a healthy baby. As a part of our Prenatal Care Coordination program, you can receive assistance once your baby arrives.

Your prenatal care coordinator will send you a customized calendar that includes important follow-up dates for your child's health checks. In addition, you will receive a welcome packet that includes useful information to help parents of newborns.
