Primary care provider assistance
Children's Community Health Plan wants to help keep you and your family healthy. Finding a primary care provider is an important way to do this. Seeing your primary care provider on a regular basis is good for your health. When you see your primary care provider for check-ups, it can prevent visits to the emergency room and save you time.
A primary care provider is your main health care provider and will:

Provide care to keep you healthy.

Teach you how to stay healthy.

Identify and take care of common medical problems.

Decide the best place for you to go when you are sick.

Make referrals to specialists when needed.
When you have a primary care provider, you can call him or her with questions about your health and he or she will know who you are.
Primary care providers include:

Family practice. These physicians see patients of all ages. Your whole family can see the same doctor. Some also care for you if you're pregnant.

Internal medicine.These physicians see adults, usually older than 18.

General pediatricians. These physicians specialize in the needs of children, usually until they reach 18 years old.

Nurse practitioners and physician's assistants. These care givers can also be your primary care provider. They offer well visits and care for you when you're sick, just the same as a doctor.
